To ensure a smooth transition home, your care team will handle your discharge arrangements, such as scheduling appointments, preparing medications, and finalising your bill.

• Lab & Radiology Reports (For e-Copies, download MyHealth360 App to retrieve 3 days after discharge)
• Post-Discharge Medication
• Discharge Documents (e.g. Medical Certificate, Discharge Summary etc.) 
• Scheduling of Follow-Up Appointment
• Billing Methods (Your care team will update you on further instructions for billing) 
• Booking of Discharge Limousine
